This is what the film is about:

It seems like ages since Sakuta Azusagawa and Mai Sakurajima found each other. But their harmonious relationship is put to the test when Sakuta’s first great love Shōko Makinohara appears and moves in with him without further ado! But that’s not all, because there seem to be two different versions of her: In addition to the adult Shōko, Sakuta also meets Shōko’s younger self, who is still in middle school and apparently suffers from a serious illness.
